When individuals complete an HRA it paves the way for them to make healthier lifestyle choices, because it gets them thinking about the risks they face and what they can do to change their lifestyle. Over time this shift in thinking translates to a shift in behavior, which reduces the amount of money that health insurance companies must pay in claims. Process measures are derived from data collected in the OASIS submitted by home health agencies and are calculated using a completed quality episode that begins with admission to a home health agency and ends with discharge, transfer to inpatient facility or sometimes death. Measures based on OASIS data are calculated using a completed episode of care that begins with admission to a home health agency and ends with discharge, transfer to inpatient facility or, in some cases, death. The Centers for Medicare & Medicaid Services continually rolls out regulations that rely on the collection of detailed electronic information, so home health agencies that don’t use EMRs optimized for interoperability risk falling behind. And with patients seeing more and more providers, being able to accurately track each patient’s health journey and easily access accurate patient information during sensitive transitions of care is more important than ever.
Technical documentation for calculating process measures can be accessed via the link to Technical Documentation of OASIS-Based Measures in theDownloadssection below. With this increased oversight, it’s all the more vital for home health agencies to effectively track, measure and report their processes and outcomes. Uncover best practices for improving your patient experience that will reflect in your CAHPS scores, reviews and referrals. Require employees and contractors to certify on job applications or in contracts that they are not excluded from federal programs and have not been convicted of offenses that would result in exclusion.
HEALTHCARE MARKETING ROI AND ATTRIBUTION
Most (90%) of the$3.8 trillionthat the U.S. spends on health care annually is spent on chronic health conditions. The CMS Office of the Actuarypredictsthis staggering figure will grow, on average, 5.5% each year, reaching $5.7 trillion in 2026. Employers and health plans cover a significant portion of their populations’ medical expenses in a highlyregulatedmarket.
Though data collection is more important than ever, uninterrupted face time between clinicians and their patients is essential to high-quality care. That’s why you don’t want to be wasting precious hours manually entering the same data over and over again. Not only that, but the more people you have entering data, the more opportunity there is for error.
How can an HRA improve population health?
A home safety assessment is an important tool for home health care in verifying the safety of patients and their home environment, identifying and correcting deficiencies, and minimizing and preventing losses. As the majority of reimbursement for home care services is through Medicare or Medicaid, the government plays a major role in setting the quality of care standards that organizations should follow. Periodic on-site inspections are performed to ensure compliance with the laws, regulations, and standards of care. The federal government works in partnership with state agencies , which often set additional regulations that govern the licensing of home health care providers. In addition, private insurers and the home care companies themselves may set quality standards of their own to follow. For Medicare coverage, home health services must be ordered and certified by a physician, the patient must be homebound, and the services must be reasonable and necessary to diagnose or treat the patient’s illness, injury, disease, or condition.
As well as being a legal requirement, you will find that many clients and main contractors will not allow you to start work until you have prepared and provided them with a copy of your method statement and/or risk assessments. A professionally prepared risk assessment can mean the difference between winning and losing a tender. Your client/main contractor will easily spot a poorly written risk assessment and this will only demonstrate that you do not understand your responsibilities and are prepared to take shortcuts when it comes to health and safety.

Documentation of the home safety assessment and any corrective actions is imperative, with any changes or substandard safety issues noted, evaluated and solved during follow-up visits. With this aggressive oversight, home health providers face compliance risks with severe criminal, civil, and financial consequences for violations. A good overview of the scope of home hazards is provided in a recently published qualitative study by Markkanen, et al.35 Data on occupational hazards were collected from HHCWs participating in focus groups and in-depth interviews. They identified general security/personal safety hazards that could present a threat to patient safety, including unsafe neighborhoods, violent or unstable patients and family members, and potentially dangerous pets. The study participants also raised environmental concerns, including overheated room temperatures, poor indoor air quality, and unsanitary conditions, such as the presence of insects and rodents. The Community Preventive Services Task Force recommends using health assessments in combination with health education programs, based on strong evidence of effectiveness in improving health behaviors in populations of workers. The CPSTF states that using HRAs with feedback can improve a wide range of health and safety concerns, such as alcohol and tobacco use, high blood pressure, and high cholesterol.
Identifying and Communicating Risk
In some cases, the personal care attendant does perform household chores and thus has more control over the situation. However, in cases where other household members perform these chores, additional training or support may be required. Policies and procedures for addressing this issue should be the subject of further inquiry and interventional studies. This is important, not only in terms of the risk that unsanitary conditions present for the transmission of infectious disease, but also because some of these hazards increase the risk of injury (e.g., falls), and some conditions (e.g., excessive clutter) are fire safety hazards. This included training on safe lifting ; the proper use of Hoyer lifts ; electrical safety ; fire safety and evacuation ; personal safety ; respiratory protection ; slip, trip, and fall prevention ; and standard precautions and infection control .
A risk assessment is the process in which you identify and evaluate any potential hazards in the workplace. If you employ five or more people, the findings from this risk assessment need to be written down and clearly communicated to your employees. Incident report data alone cannot provide an inclusive picture of an organization’s activities and potential exposures.
One of the AKS’s purposes is to ensure that healthcare providers do not enter into certain relationships with other entities or individuals that are intended to induce the referral of beneficiaries of federal health plans. Specifically, the AKS prohibits a person or entity from knowingly or willfully offering, soliciting, paying, or receiving remuneration to induce federal healthcare program referrals. “Remuneration” refers to anything of value paid directly or indirectly, in cash or in kind, which is intended to induce the referral of patients. Such remuneration can include cash, free goods or services, discounts, gifts, below-market rent, or relief of financial obligations. As such, the federal government keeps a keen eye on home health practices that put strain on the public health system or put patients at risk. Department of Health & Human Services , the Department of Justice, and various state agencies aims to protect government healthcare plans and their beneficiaries.

The following year theCenters for Disease Control and Prevention released publicly available HRA software that included a self-administered survey to calculate adult health risk. Healthcare marketers and patient navigators use health assessments to identify prospective patients, engage with them to seek medical care, and ultimately schedule patient encounters. CMS usually updates the HH QRP claims-based measure results every year. However, due to the COVID-19 Public Health Emergency HHQRP data submission requirements for the Q4 2019, Q1 2020, and Q quarters were exempted.
